[PVFS2-developers] Checked in changed to request code

OK, the changes I made are checked in.  As far as I can tell everything
still works as it did before, only now we have a resized constructor,
and it works correctly - including negative extents, and requests whose
size equals the extent, but the request is not contiguous.

Obviously this is a good time to run a the standard set of tests and be
on the lookout for anything thats not right.
