Just now I changed the code to avoid killing the server when this
happens.  Although the error itself is pretty critical, maybe it
is possible to continue---server state has not necessarily been
corrupted even though this one client is awry.

Also changed the error message a bit to explain what was meant
by that error.

Hopefully something obviously wrong will appear in the changed
in-progress operation handling as Sam was mentioning.
